h**a 发帖数: 108 | 1 被告知肺部有血栓但是不是大问题,但是看到以下的医院的CT报告又觉得比医生说的要
严重一些,不知哪位能帮忙解读一下。万分感谢
Findings: There is significant thrombus within the right lower lobe
pulmonary artery, which distends the artery somewhat and is nearly occlusive
. There are several vessels distal to this, which also appear to contain
thrombus. Contrast does, however, pass beyond this into the more distal
branches. There is what appears to be peripheral consolidation, which has a
lobulated, slightly ill-defined contour, which could represent focal
infarction in the lung periphery supplied by the embolized vessel. A small
effusion is seen. The lungs are otherwise clear. There is no adenopathy. The
aorta is unremarkable as is the heart. The visualized upper abdominal
structures are normal. | f****o 发帖数: 2770 | 2 what did your doctor tell you exactly?
what treatment will you receive?
radiology evaluation could be very vague. | c********e 发帖数: 496 | 3 first, I guess this is a spiral CT report, is that right? regular CT plays
no role in the Tx of PE.
pulmonary embolism sounds horrible, actually it is not uncommon though,
while most of the cases shows no symptoms, until a major vessel is blocked.
If you are free of symptom, all you should do is to figure out the source of
the embolus, especially rule out DVT, the most common one, a Doppler may
help with it.
If you have SOB, tarchycardia, or chest pain, stay in the ER, heparin (
preferred) or LMWH is the drug of choice, warfarin is warranted. There is a
complicated guideline for the treatment of PE, refer to your doc. | h**a 发帖数: 108 | 4 Thanks for the replies. From the report, it shows hospital used 64-slice CT,
not sure it is the same as spiral CT.
Even though doctor told me it is not serious but she still put me on lovenox
for one week and coumadin for 3 months.
At the beginning I thought the blood clot is in vein but report shows it is
in artery. Which is more serious, in vein or in artery? How important is the
right lower lobe pulmonary artery?
One more piece of info from the report:
IMPRESSION: The study is positive for pulmonary emboli within the right
lower lobe pulmonary artery and several of its more distal branches. A
peripheral infarct is suspected. This could also be infectious, which is
considered less likely. | c********e 发帖数: 496 | 5 64 slice CT is kind of spiral CT, more advanced than 16 slice CT.
It is pulmonary artery, instead of vein for sure.
Administration of lovenox and warfarin is reasonable, unless you have other
risk factors. Note you need to check your CBC on your next visit,to monitor
platelet level, decrease of which is one of major SEs of LMWH.
As I said, if you do not have any symptoms, it is fine. Watch out, if you
have swelling or redness on your legs, do see a doctor immediately. | s*****a 发帖数: 258 | |
